1. The boat must have a value. This value must
be higher than what we charge for the service. This must be written on
the form. This price will also be on the bill of sale that will be sent
back to you. Fair market value should be considered when determining the
price.
2. Affidavit and bill of sale must be notarized.
3. The printed name and drivers license number must be legible and
accurate.
4. Hull (HIN) number must be legible.
5. Please print your full name and address or if title is to be in
another name, please print information below.
6. Check or money order must be payable to International Title
Service.
7. If paying by credit card, fill out line 8.
