INSTRUCTIONS

ALL FORMS MUST BE COMPLETED
1. Power of Attorney must be notarized and have your date of birth. We must have the original, no copies.
2. The person who completes the Power of Attorney must be the same
as the person whose name is to appear on the registration. Only one
power of attorney needs to be completed if all the transactions will
have the same name on the registration. If sending multiple transactions
in different names, a power of attorney must be filled out separately
for each name to appear on registration. The power of attorney will be
kept on file and can be used to process any future transactions under that name. We cannot process in a business name.
3. The serial (VIN) number must be legible.
4. Copy of current insurance card for vehicle being registered
indicating your name, year, make, serial number, effective and
expiration dates. The insurance must be valid for a minimum of 60 days from the receipt of this form. If you do not have insurance, contact your current company. We cannot process in a business name. This does not apply to trailers.
5. Bill of Sale showing your name, year, make, serial (VIN) number,
price, signature and address of the seller. Bill of Sale must be
notarized, no copies.
6.Must have photocopy of driver's license.
7. Check or money order must be payable to International Title
Service.
8. If paying by credit card, complete line 9.
